How to Fill Out Forms

This section provides information for the completion of two forms, the
application and the production header/summary sheet.

HOW TO FILL OUT THE APPLICATION FORM - NYQL

Please use blue or black ink and write legibly.

NAME: As it should appear on the Qualification List.

ADDRESS: Where the DGACA-East can mail official placement notification.

SOCIAL SECURITY #: Last four digits only

CELL PHONE #: Where the applicant can be reached during the day to answer questions about his or her application.

CLASSIFICATION APPLYING FOR (check one): Second Assistant Director (2nd AD), First Assistant Director (1st AD), Multi-Camera First Assistant Director (MC1st AD), or Unit Production Manager (UPM).

INITIAL/UPGRADE/ELIGIBLE/INTERCHANGE APPLICATION (Indicate one):

Initial Application - Check if you are not currently on any New York Area QL
Upgrade Application - Check if you are currently on the New York Area QL and want to 'upgrade' your classification on the list (i.e., from 2nd AD to 1st AD).
Eligible Application Check if you are currently on a New York Area QL and are submitting additional workdays in the same classification to be eligible to work in the Southern California Area.
Interchange Application - Check if you are currently on the New York Area QL and are submitting additional workdays in the same classification to 'interchange' (transfer) to the Southern California QL.

For each project you have worked on (beginning with the most current) indicate:

PROD TITLE: Production Title (working title and, if different, release title)

PROD CO: Production Company/Studio

DIRECTOR: Director's Name

UPM: Name of the Unit Production Manager

AD's/SM's: Names of ALL Assistant Directors or Stage Managers beginning with the First ADs

CAM: i.e., Single or Multi-Camera

MEDIA: i.e., Film, Tape

TYPE: i.e., Feature, Episodic, TV Movie, Special, etc.

JOB TITLE: Your job title. If you worked in more than one job classification, indicate the number of days worked in each classification.

DATES OF EMPLOYMENT: From the first day of work to (including) the last day.

NUMBER OF WORK DAYS: Divide the number of days worked into three categories: prep, shoot, or wrap. Divide shoot days into two categories: studio or distant location.

  • Paid travel days are counted as prep days.
  • Camera blocking days and camera test days are counted as shoot days.
  • For placement as a Multi-Camera First Assistant Director every workday of a week in which shooting occurs is counted as a shoot day.
  • Distant location is a location on which an employee is required by the employer to remain away from home and be lodged overnight.
  • Applicants are not credited for any work without proof of payment.
  • Days paid but not worked (i.e. holiday, hiatus, idle location day) are not credited.
  • One day of work is credited when an applicant is paid for two jobs in one day.
DOCUMENTATION: Check each form of documentation included (Deal Memo, Pay Records, etc.) and identify any other documentation that is enclosed.

TOTALS: Total the Number of Work Days on each page, and indicate the Grand Total of all days worked on the first page.

25%/75% The requirements for placement state that 75% of days worked must be spent with the actual shooting company. Therefore, 75% of the minimum days required must be shoot days.

This means:
If the requirement is a minimum of 520 days, at least 390 of the 520 days must be shoot days.

If the requirement is a minimum of 350 days, at least 263 of the 350 days must be shoot days.

If the requirement is a minimum of 260 days, at least 195 of the 260 days must be shoot days.

If the requirement is a minimum of 200 days, at least 150 of the 200 days must be shoot days.

Note: Days credited to an applicant for placement on any DGACA-East Qualification List are generally carried forward when the applicant upgrades (changes classification on that list). However for determining eligibility to work in the Southern California Area or to interchange to a Southern California Area Qualification List, some workdays may not be counted for this purpose.

OTHER: Clarification or additional information regarding work on this project
DISTANT LOCATION: Work dates on location, city, state (include country if work is outside of the United States)
